School Meals

Academy of Math and Science Glendale is offering healthy meals to all students at NO COST every school day in School Year 2023-2024.  Your student(s) will receive free breakfasts and lunches regardless of your household’s income.  Breakfast in the Classroom will be served for grades K-1 and in the gym for grades 2-8 from 7:30 AM -8:00 AM.  If your student is participating in after-school activities, snacks are provided at no cost too.
NO APPLICATION for Free and Reduced-Price School is required if you completed one already during this school year.   
 
However, if your student is new or you did not complete an application before 4/1, it is essential that you complete an ESEA (Title 1) Income Eligibility form because your school’s:
 
  • Federal and state educational funding and lower internet charges are based on data learned from this form
  • Federal funding for school meals is based on data from the form.
  • An ESEA (Title 1) Income Eligibility form is available and should be returned to your school’s office manager or registrar.

Description:

SFA- Academy of Math and Science Desert Sky; Academy of Math and Science Glendale; Academy of Math and Science Peoria Advanced desires to contract with a Caterer to provide meals for our SFA’s food service program.  Offers received by the correct time and date will be opened and the name of each Offeror will be publicly read.  Offers must be in the actual possession of Academy of Math and Science Desert Sky; Academy of Math and Science Glendale; Academy of Math and Science Peoria Advanced on or prior to the time and date, and at the submittal location indicated.  Late offers will not be considered.  Offers must be submitted in a sealed envelope or package with the Solicitation Number and the Offeror’s name and address clearly indicated on the envelope or package.  Proposals will be accepted until May 28, 2024, 10:00 am at 3002 S. Priest Drive, Tempe, 85282.

If you do not provide the information or provide incomplete information, your children may no longer receive free or reduced-price meals.

The letter must contain the complainant’s name, address, telephone number, and a written description of the alleged discriminatory action in sufficient detail to inform the Assistant Secretary for Civil Rights (ASCR) about the nature and date of an alleged civil rights violation. The completed AD3027 form or letter must be submitted to USDA by:
